Sport Sign-Up
PREVIOUS YEAR SPORTS ROSTERS DO NOT CARRY OVER TO THE NEXT SCHOOL YEAR.
ALL STUDENTS NEED TO BE SIGNED UP AND CLEARED PRIOR TO THE START OF TRYOUTS.
The following requirements must be completed in order to be eligible for participation:
- Physical Evaluation - To participate in sports, student-athletes submit to have a physical evaluation every 13 months (12 months with a one-month grace period). Once the form is completed by your child’s healthcare provider, please upload it to your child’s profile in the Mo‘omō‘ali Olakino (EHR) Parent Portal.
